If you wish to change your name from that used when you first enrolled with York St John University you must submit a Change of Name form and show supporting evidence that the new name is valid, such as a birth or marriage certificate, passport, or deed poll.  If you are an international student you must provide your passport.

The Change of Name form can be found on the website and the completed form and relevant documents should be sent to Student Records or taken to the Student Information Desk in Holgate.
